Welcome to the online membership application form

To apply for CIQS membership, please complete the online application form and make payment for the non-refundable application fee to submit your application for review.  You will be required to upload supporting documents such as your academic records, resume, etc.  Do NOT upload any documentation with your private and confidential information, such as passport numbers, salary, etc.  Applications will be submitted to the CIQS Membership Committee for assessment to determine membership category and the Test of Professional Experience (TPE) requirements to achieve professional designation (CEC / PQS) ONLY when all required supporting documents have been received and payment has been paid. 

We do NOT accept applications via email.
